***Very Important -If you are electing dental coverage for less than your entire family, you will need to have your PACID fax a notice to the DEERS Research and Analysis Office at 831-655-8317. Their regular phone number is 1-800-361-2508. You will need to advise them that you are electing dental coverage for specific family members only (List the name and SSN of each family member that is to have dental coverage on the fax). DEERS will change your DEERS record to reflect only those you have elected to show dental coverage. When United Concordia inputs your dental coverage plan, they check the DEERS record for eligibility. If you have more family members listed than you chose to cover, your dental coverage may be rejected until both DEERS and United Concordia match on who is supposed to be covered.

For general information, call United Concordia at 1-800-866-8499, or visit United Concordia's Web site http://www.ucci.com

Effective 1 Feb 01, you get more benefits for less money.

AFTER ENROLLMENT

You need to ensure that you and your family members are correctly coded in DEERS to take advantage of this fine benefit.

Active duty and ARNG M-day soldier’s need to check their LES each month to ensure that an allotment is reflected for DENTAL. If you see an entry one month and no entry the next month, please give United Concordia a call or email them reference the change ASAP. You could have been dis-enrolled for some reason and the quicker you notice it the less impact it will have on your coverage.
